Medication Reviews
ALL PATIENTS ON REGULAR MEDICATION ARE REQUIRED TO ATTEND FOR A MEDICATION REVIEW WITH THE CLINICAL PHARMACIST EVERY 12 MONTHS (AT LEAST). THIS IS REGARDLESS OF BEING UNDER A CONSULTANT, ETC.
IF YOU FAIL TO RESPOND TO THREE INVITES FOR A MEDICATION REVIEW YOUR MEDICATION WILL BE TAKEN OFF REPEAT AND WILL NOT BE AVAILABLE FOR RE-ISSUE BY A MEMBER OF THE RECEPTION TEAM.
PLEASE CHECK THE RIGHT-HAND SIDE OF YOUR PRESCRIPTION FOR ANY MESSAGES.

Patient Participation Group
The practice runs a Patient Participation Group and the door is always open to new members. If you are interested please contact the surgery with your details. Don't worry if time is a problem for you - you can become a virtual member whereby discussions could take place via email rather than in person. More information is available here
GP Allocation
All of our patients are allocated a named, accountable GP. This does not affect your wishes to see a preferred GP. Please contact reception for more details.
Data Protection
Under the current Data Protection Act all practices are required to have a Fair Processing Notice (FPN) which informs patients why you use their information and their rights.

Data Protection Policy
National Data Opt Out Programme
NHS Digital has developed a new system to support the national data opt-out which will give patients more control over how confidential patient information is used. The system will offer patients and the public the opportunity to make an informed choice about whether they wish their confidential patient information to be used just for their individual care and treatment or also used for research and planning purposes.
Patients and the public who decide they do not want their confidential patient information used for planning and research purposes will be able to set their national data opt-out choice online.
