ALL PATIENTS ON REGULAR MEDICATION ARE REQUIRED TO ATTEND FOR A MEDICATION REVIEW WITH THE CLINICAL PHARMACIST EVERY 12 MONTHS (AT LEAST). THIS IS REGARDLESS OF BEING UNDER A CONSULTANT, ETC.
IF YOU FAIL TO RESPOND TO 3 MESSAGES, YOUR MEDICATION WILL BE TAKEN OFF REPEAT AND WILL NOT BE ABLE TO BE ISSUED BY A MEMBER OF STAFF IN RECEPTION.

Patient Participation Group
The practice runs a Patient Participation Group and the door is always open to new members. If you are interested please contact the surgery with your details. Don't worry if time is a problem for you - you can become a virtual member whereby discussions could take place via email rather than in person. More information is available here
All of our patients are allocated a named, accountable GP. This does not affect your wishes to see a preferred GP. Please contact reception for more details.

NATIONAL DATA OPT OUT PROGRAMME
NHS Digital has developed a new system to support the national data opt-out which will give patients more control over how confidential patient information is used. The system will offer patients and the public the opportunity to make an informed choice about whether they wish their confidential patient information to be used just for their individual care and treatment or also used for research and planning purposes.
Patients and the public who decide they do not want their confidential patient information used for planning and research purposes will be able to set their national data opt-out choice online.

Please note: Prescription Requests
The surgery no longer accepts repeat prescription requests over the usual telephone lines, as we need to keep these free for other, ever growing medical needs. All future prescriptions can be ordered on the 24 hour answer machine on 01457 835345, dropped in at reception or ordered on-line (please ask to register for this at reception or see below). We apologise for any inconvenience this may cause.

Please note that if you already order your prescriptions on-line, you will now need to re-register as this is a new system with the addition of on-line appointment booking included. Current online patient services
This practice currently offers patients’ facilities to book, view, amend, cancel and print appointments online.
This practice currently offers the facility for all patients to order online, view and print a list of their repeat prescriptions for drugs, medicines or appliances.
This practice plans to offer patients facilities to view online, export or print any summary information from their record, relating to medications, allergies, adverse reactions and any other items agreed between the practice and individual patient, from March 2015.
These dates are subject to the necessary NHS GP systems and software being available to the practice by NHS England.
We will publicise and promote our online services to our practice’s patients through the practice website, practice waiting room leaflets, posters and the Patient Participation Group by September 2014.
